In Gloucestershire, we know that every woman and her family is unique with their own needs and wishes. We also know that it is important for you to have a say in the care you receive during your journey to parenthood. To support you with this, we have worked with local women to develop a Personal Care Plan. This will help you to explore, understand and record your choices and preferences during your pregnancy, birth and the first few weeks of parenthood. Personal care plans are for everyone, whether it is your first baby, second, third or more!

Completing your journal and setting out your goals and wishes throughout your journey will help you to prepare and plan what’s important to you. Work through the journal at your own pace and discuss with your maternity team at any time.

In response to feedback from women in Gloucestershire, the journal is available digitally, giving you flexibility in the choices and decisions you make. It is important to remember that a plan is just that – and that things may need to be reviewed and changed around your needs and the needs of your baby. We must ensure your care and the care of your baby is always high quality and safe.

The Journal can be printed and hand written or downloaded and completed on your phone or tablet. Within the journal you will find seven sections to support you throughout key stages within your pregnancy. Bring the journal along to your maternity appointments and remember to share with your maternity team.


Saving your Journal onto an iPhone

  • Open the Journal using the button above
  • Select
  • Swipe through the middle row, then select Copy to Books.
  • Open Books (sometimes stored in Misc folder) and find your Journal stored in your Library.

Saving your Journal onto an Android

  • Open the Journal using the button above
  • Tap the three vertical dots icon on the top-right
  • Tap Print.
  • Tap Select printer.
  • Tap Save as PDF.
  • Tap the Save icon
  • Now you can select the place where you want to save the file and tap Save.
